Job Description

Blize Healthcare is looking for a Registered Nurse or Licensed Vocational Nurse to join our team.

 
You will provide routine healthcare to patients at the patient's home or in a care facility in San Francisco County.

Job Description:

• Responsible and accountable for the delivery and coordination of patient care and the application of the nursing process, during after-hours and weekends.
• Demonstrates the ability to make clinical judgments and utilize critical thinking in an effective and efficient manner with supervision.
• Perform according to nursing standards and is accountable for managing patient care and assisting others in the management of patient care. 

Required Skills & Qualifications:

• Must have a minimum of 1 year of experience as an RN or LVN.
• Provide skilled nursing services/interventions in accordance with physician orders.
• Assesses patient needs plans for evaluation and modifies care to meet goals of skilled nursing interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
• Experience in home health or hospice is recommended.

• Driver's License (Required)

• LVN must be IV Certified (Required)
