Join Our Team as a Remote Full-Time Primary Care Nurse Practitioner

We are currently seeking a  Remote Full-Time Primary Care Nurse Practitioner to join our growing team.

Position Overview

We are looking for a skilled and compassionate  Primary Care Nurse Practitioner (NP)to provide virtual healthcare services to a diverse patient population. This role is fully remote, offering the flexibility to deliver high-quality care from the comfort of your home while making a meaningful impact on patient health.

Key Responsibilities

• Conduct virtual patient assessments, diagnose acute and chronic conditions, and develop treatment plans
• Prescribe medications and recommend treatment options in accordance with best practices
• Perform remote patient monitoring and provide follow-up care as needed
• Educate patients on disease prevention, health maintenance, and treatment adherence
• Collaborate with a multidisciplinary team, including physicians and specialists, to ensure comprehensive care
• Maintain accurate and up-to-date electronic medical records (EMRs)

Qualifications

• Master’s degree or higher in Advanced Practice Registered Nursing (APRN)
• Active and unrestricted  Nurse Practitioner (NP) license in New York State
• National certification as a Nurse Practitioner (CRNP) with prescriptive authority
• Experience in telehealth or remote patient care preferred
• Strong knowledge of medical terminology, pharmacology, and primary care practices
• Excellent communication and interpersonal skills to engage effectively with patients and colleagues
