Job Description

As a Part Time Registered Nurse (RN), you will be an ECM Clinical Team Leader at Heritage Health Network, you will play a pivotal role in leading a multidisciplinary team dedicated to improving health outcomes for high-risk members through the Enhanced Care Management (ECM) program. This role requires strong leadership, medical file oversight, and team collaboration to ensure the effective integration of medical, behavioral, and social services.

This role combines remote work, in-office responsibilities, and team collaboration engagements, with work arrangements tailored to meet program and member needs.

Requirements

Key Responsibilities:

  • Leadership and Supervision: Provide clinical leadership within a team of three, including a Behavioral Health Care Manager, a Lead Care Manager, and a Community Health Worker, ensuring a holistic approach to patient care.

  • Clinical Caseload Management: Review member files to ensure quality care and clinical compliance for best medical practice

  • Comprehensive Care Planning: Conduct risk assessments and develop individualized Patient-Centered Care Plans in collaboration with Lead Care Managers

  • Healthcare Provider Collaboration: Build partnerships with healthcare providers and community organizations to facilitate referrals and access to community resources.

  • Transitional Care Coordination: Coordinate with hospital staff on discharge plans and transitional care needs as requested for your Lead Care Manager team members. 

  • Regulatory Compliance and Documentation: Ensure compliance with healthcare regulations, maintain accurate documentation in electronic health records (EHR), and contribute to monthly reports for health plans.

  • Member Education: Act as a primary point of contact for educating members and their families on health-related issues.

Qualifications:

  • Licensure: Active Registered Nurse (RN) license in the state of California.

  • Experience: At least 3 years of nursing experience, with a minimum of 1 year in a leadership or care management role.

  • Specialized Knowledge: Experience in care coordination and managing complex patient populations, preferably within an ECM or similar program.

  • Skills: Strong clinical assessment, care planning, and coordination skills.

  • Interpersonal Skills: Excellent communication and teamwork skills, essential for working effectively within a multidisciplinary team.

  • Regulatory Knowledge: Familiarity with healthcare regulations, Medicare/Medi-Cal guidelines, and care management best practices.
