Job Description

Position Summary

This job is for a non-profit healthcare clinic that is always looking to help our their communities, regardless the situation. They cover all chapters of life, but this position is exclusively working within PACE, the geriatric department. Also, the Physical Therapist position pays $50 - $62.25 per hour with a $5,000 sign-on bonus!

The Physical Therapist contributes to the mission and vision of our organization by delivering comprehensive physical therapy services to our participants. This role involves performing clinical evaluations, developing personalized treatment plans for acute, restorative, and maintenance levels, and incorporating these plans into the participants' overall care as prescribed by healthcare providers. 

Key Responsibilities

- Assess and determine participants' rehabilitation needs at various levels, following the guidelines set by the American Physical Therapy Association (APTA).
- Collaborate with staff in nursing homes and assisted living facilities to address the rehabilitation needs of participants.
- Maintain accurate documentation of participants' care plans, including referrals to other services such as occupational therapy, speech therapy, and medical care.
- Communicate participants' positioning, mobility, and safety needs to staff as required.
- Liaise with family members and caregivers to ensure they understand and can support the recommended therapy plans.
- Provide physical therapy treatments, including therapeutic exercises, modality treatments, and functional mobility training.
- Supply participants and caregivers with detailed instructions and visual aids for home exercises when necessary.
- Ensure participants and caregivers are fully informed about the terms and policies of the durable medical equipment (DME) loan program.
- Delegate therapy tasks to support staff based on the complexity of the participants' needs and procedural requirements, adhering to state regulations.
- Educate participants and caregivers on the use and upkeep of DME, focusing on enhancing participants' mobility, safety, and independence.
- Conduct home visits and educational sessions as outlined in the care plan, documenting all interactions and responses in the participants' records.
- Work with the multidisciplinary team to integrate physical therapy into the overall care plan for each participant.
- Participate in interdisciplinary team meetings, peer reviews, and quality management activities.
- Record all procedures and treatments in participants' medical records following organizational policies.
- Provide guidance to staff in line with state laws and organizational policies.
- Manage the ordering, inventory, distribution, and maintenance of DME for participants.
- Orient and train contract physical therapists on organizational systems and objectives.
- Identify and address any issues with DME, including repairs and replacements.
- Ensure all DME transactions, including loans, returns, repairs, and purchases, are properly documented.
- Make purchasing decisions for participants and rehabilitation operations within budgetary constraints and departmental goals.
- Encourage staff to contribute ideas for continuous improvement and efficiency in the rehabilitation department.
- Provide training to non-rehabilitation staff, such as CNAs and drivers, regarding participant needs.
- Engage in interdisciplinary team planning to develop and monitor participants' care plans.

Qualifications

- Doctorate in Physical Therapy (DPT) required.
- Graduation from a physical therapy program accredited by the American Physical Therapy Association (APTA), Committee on Allied Health Education, and American Medical Association (AMA).
- Valid and current Physical Therapist license in California required.
- Minimum of one year of experience in a geriatric physical therapy setting required; two years preferred.
- Bilingual in English and Spanish required.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) required.

Skills and Abilities

- Exceptional verbal and written communication skills, including strong composition, typing, and proofreading capabilities.
- Ability to interpret diverse instructions presented in written, oral, diagram, or schedule forms.
- Knowledge and experience in occupational therapy and speech-language pathology as they pertain to geriatrics.
- Proficiency in applying mathematical concepts such as fractions, percentages, ratios, and proportions to practical scenarios.
- Skilled in data analysis and report generation, capable of drawing logical conclusions from statistics.
- Understanding of financial management, including profit and loss, and budget handling.
- Strong multitasking abilities, with excellent planning and organizational skills.
- Able to work both independently and collaboratively as part of a team.
- Ability to manage highly confidential information professionally and ethically.

Physical Requirements

- Ability to lift and carry up to 50 lbs.
- Ability to stand for prolonged periods.
