Position Overview
We are seeking a dedicated Field Registered Nurse Float to join a five-star rated Home Care agency. In this role, you will be responsible for providing comprehensive skilled nursing care to patients in their homes across the five boroughs of NYC and Westchester County. This position requires a compassionate and autonomous clinician skilled in assessment, care planning, and patient education.

Qualifications
~ Education: Graduate of an accredited school of nursing
~ Licensure: Active and unrestricted New York State RN license
~ Experience: Minimum 1 year of hospital experience required (home care a plus)
~ Technical Skills: Proficiency with EMRs; HCHB experience preferred
~ Soft Skills: Strong assessment, critical thinking, communication, and time-management skills; self-directed and independent
~ Other: Valid drivers license; must be willing to travel across 5 boroughs + Westchester County
Key Responsibilities
~ Perform home care assessments to determine patient eligibility
~ Complete comprehensive assessments in HCHB and develop Plans of Care with patients, families, and physicians
~ Provide skilled nursing care (wound care, injections, medication management, disease management, etc.)
~ Supervise and orient home health aides per regulations
~ Evaluate effectiveness of interventions and update plans of care as needed
~ Collaborate with interdisciplinary team (PT, OT, MSW, speech, nutrition)
~ Educate patients and caregivers on disease management and treatments
~ Maintain timely and accurate documentation in HCHB
~ Manage caseloads including MD communications, case conferences, interim orders, recertifications, and discharge planning
~ Review and complete outstanding documentation daily within 48 hours
~ Participate in QI activities, mandatory in-services, and interdisciplinary case conferences
~ Follow infection control principles and agency standards
~ Perform other duties as assigned
