Requirements of a Field Registered Nurse:

~ Home Care Home Base EMR experience is a must.
Schedule of a Field Registered Nurse:
~ Full-time position.
Compensation and Benefits:
~ Competitive salary ranging from $105,000 to $110,000 per year.
~ $15,000 Sign-On Bonus after 15 months.
~ 4 weeks of vacation.
~ 5 personal days.
~ 8 sick days per year.
~ Mileage reimbursement.
Job Description of Field Registered Nurse:
The Coordinator of Care is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This includes conducting home visits, completing required documentation, communicating with PCPs, completing 485 and interim orders, updating medication profiles, care coordination with other disciplines, reviewing case communication notes, addressing follow-up items and incomplete documentation in HCHB, and participating in case conferencing with the respective supervisor.
Responsibilities of Field Registered Nurse:
~ Performs a home care assessment to determine the patient's eligibility for services.
~ If not eligible|appropriate for home care services, indicates not admitted in the system with reason for determination.
~ Completes a comprehensive assessment utilizing HCHB when appropriate.
~ In conjunction with the patient's family and physician, develops and implements the Plan of Care based on a comprehensive assessment.
~ Provides skilled nursing care as described in the 485-Plan of Treatment (wound care, injections, prepour|prefill of medications, disease management, medication management, etc.).
~ Documents in HCHB Clinical Software all assessments, treatments, services provided, and patient responses.
~ Case manages respective caseload, ensuring communication with MD, case conferencing, updates from interdisciplinary team, interim orders, updating patient profile, and conducting recertification assessments.
~ Completes all assessment visits within 48 hours of the assessment date.
~ Reviews incomplete|follow-up items daily in HCHB and prioritizes them before visits; checks twice daily for updates.
~ Completes 60-day summary on recertification 485 (locator 22 area).
~ Conducts discharge planning and communicates anticipated discharge date with patient, family, physician, and interdisciplinary team.
~ Observes Infection Control principles and adheres to standard precautions at all times.
~ Performs all other duties as assigned.
