Your Specific Responsibilities:
This is a paraprofessional position responsible for promoting and providing prenatal and infant social services and education to women and their families eligible for the Healthy Start Program. Certain immunizations and TB testing may be required.
Implements the Healthy Start Program to families referred to and enrolled in Healthy Start in a manner consistent with the Healthy Start Standards and Guidelines and the Healthy Start Coalition Contract. Responsible for outreach, initial assessments (IAs), and assisting participants with access to prenatal or infant care and community resources (i.e. social, medical, or educational services). Provides ongoing care coordination services, screenings and education including smoking cessation, parenting, substance abuse, domestic violence and breastfeeding support/education. Education components may require added training (i.e. RAT, Edinburgh, SCRIPT, SBIRT, and FSU Parenting Curriculum, etc.). Provides or arranges services in a location that best meets the needs of the family (i.e. home visits). Monitors Healthy Start participants’ access to prenatal and infantservices. Participates in service delivery planning with a Registered Nurse (RN) and supervisor.
Assists with Coordinated Intake and Referral (CONNECT) program in completing initial intakes in client’s electronic record to connect pregnant women, inter-conception women and families of children under 3 to services to offset risk factors that may lead to poor pregnancy outcomes. Provides information and referrals for community maternal-child home visiting programs available to clients. Provides follow-up to the healthcare provider, the child’s primary care provider, or the referral source.
Documents care coordination services and maintains/updates participant files per Healthy Start Standards and Guidelines via the Well Family System (WFS) web-based site. Performs Quality improvement functions.Completes HMS data entry of Prenatal Screens.
Acts as a liaison with community agencies and service organizations through referral and transitional services. Attends scheduled meetings, training sessions, and outreach events. Promotes public awareness of the Health Start Program and develops and maintains community relationships to support participant referrals.
Performs other related duties as assigned.

Knowledge of community resources for at-risk pregnant and interconception women, infants, young children, and their families and procedures for accessing them, and knowledge of maternal and child health principles and standards of care. The ability to advocate on behalf of clients, communicate verbally and in writing with health care providers and other community resource people, work with children and families in their home environment, work independently and stay on task, organize/coordinate work assignments, open and check emails daily, use Microsoft Office computer programs and online data systems, actively listen and effectively communicate, work with culturally diverse families, establish and maintain professional relationships, maintain professional boundaries, accept individual differences, adapt to change, travel throughout Hendry and Glades Counties, and maintain a flexible schedule to meet the needs of participants served.
